Residential Services

Residential Habilitation (RHS)

we provide up to 24 hour a day supports and services. RHS is designed to meet each individual’s unique needs to ensure health and wellness and to support acquisition, improvement and retention of skills necessary to live successfully in one’s own home.

Community Integration and Habilitation (CIH)

are services outside the individual’s home intended to build relationships and increase independence and overall quality of life. We focus on socialization, self-care, sensory/motor development, daily living skills, communication, community living and social skills.

Participant Assistance & Care (PAC)

programs support individuals to remain and live successfully in their own homes and to function and participate in their communities. We support individuals in activities of daily living, self-care, mobility and welfare to increase their independence at home.

Respite Care

a non-goal-based service that gives primary caregivers a break to complete activities that they need to, without having to worry about their loved one. Respite services provide trained staff on a short-term basis to support participants while the primary caregiver is unavailable. Respite services are typically provided in the family’s home but may also take place in a community setting.

Wellness Coordination

Wellness Coordination includes the oversight and development of a Wellness Coordination Plan, coordination and training of staff on client’s health care needs, consultation with Physicians, Specialists, and other medical personnel, face-to-face consultations with the clients, including quarterly and annual physical assessments, consultation with support team members and active involvement in team meetings.
